{"title":"Les ondes de choc à Dudelange","content":"**Extracorporeal shock waves deliver brief acoustic pulses to a tendon, a fascia or a calcification. At the Pôle Equilibre & Santé practice, Bloc A, 48 Rue de la Libération in Dudelange, we use a radial Swiss DolorClast and a PiezoClast / DolorClast Focused. They complement a loading programme: they never replace it.**\n\n## Radial or focused: two depths, one goal\n\nThis is not electricity. A gel couples the head to the skin; the pulse is mechanical. The radial **Swiss DolorClast** (ballistic) spreads energy over a wider area, useful to about 4–5 cm — plantar fascia, superficial tendons, certain myofascial points. The **PiezoClast** (focused / piezoelectric) concentrates energy deeper, notably on a shoulder calcification or an insertion that is hard to reach.\n\nThe choice depends on depth, useful imaging, your tolerance and the goal of the day — not on a race for maximal intensity.\n\n## What we treat, what we do not promise\n\nThe best-documented situations remain **plantar fasciitis** (first-step pain) and certain **shoulder calcifications** when clinical findings and imaging agree. Patellar, epicondyle, cuff: possible case by case, always as an adjunct to progressive loading. A tendinopathy that is still very hot is generally not the right timing. Achilles is not presented here as a routine indication.\n\nShock waves do not “break” a tendon. They can modulate pain and influence the local metabolism of a chronic tissue. The foundation remains loading work, linked to [sports physiotherapy](/dudelange/competence/kinesitherapie-du-sport) or [orthopaedic rehabilitation](/dudelange/competence/reeducation-orthopedique).\n\n## Before the first series of pulses\n\nClean skin, clothing that uncovers the area, prescription and imaging reports. Mention pregnancy, local infection, unconsolidated fracture, tumour on the site, severe coagulation disorder, a pacemaker if the area is nearby, or unexplored acute pain. A light meal is enough. Avoid a thick cream.\n\nCommon contraindications: pregnancy over the area, infection or open wound, severely altered coagulation, local malignant tumour, open epiphysis in a child — the assessment rules them out.\n\n## During the session, then the following days\n\nGel, head applied, intensity raised with you. Sensation of dry knocks, sometimes unpleasant but controlled: we lower or move if it is too much. Often **a few minutes** per site, within a session of about thirty minutes that mainly contains exercises and loading advice. Three to five spaced sessions are common; the rhythm is decided according to the response.\n\nAfterwards: redness or local sensitivity for 24–48 h is possible. Brief ice if needed. Driving is generally possible. A match the same evening is not reasonable. Desk work: according to pain. Sport and physical jobs: the criteria remain strength, control and medical advice — not the number of shocks received.\n\nResults: relief may appear within a few sessions for certain selected fasciitis or calcifications. A lasting change is judged over weeks, with the loading programme. If nothing moves after a well-conducted trial, we stop.\n\n## Frequently asked questions\n\n<details>\n<summary>How should I prepare and what should I wear?</summary>\n\nAccessible area, clean skin, prescription and useful imaging. Mention pregnancy, infection, coagulation disorder or new acute pain.\n\n</details>\n\n<details>\n<summary>How long does the application last?</summary>\n\nA few minutes per site, within an overall session of about thirty minutes. The rest of the time goes to exercises and the loading plan.\n\n</details>\n\n<details>\n<summary>Is it painful?</summary>\n\nDry knocks, dosed with you. Tolerable discomfort: we continue. Sharp or intolerable pain: we lower immediately.\n\n</details>\n\n<details>\n<summary>How many sessions are needed?</summary>\n\nOften three to five, spaced out. The number depends on the response, not on a fixed package. Without clear benefit, we interrupt.\n\n</details>\n\n<details>\n<summary>Can I drive, and when can I resume sport?</summary>\n\nDriving generally possible. Sport: after assessment of load and pain, not on the evening of an intense session.\n\n</details>\n\n<details>\n<summary>What are the contraindications, and is it reimbursed?</summary>\n\nPregnancy over the area, infection, unconsolidated fracture, local tumour, severely altered coagulation: the assessment rules them out.
