{"title":"Le renforcement iso-inertiel à Dudelange","content":"**Iso-inertial strengthening uses a flywheel: you accelerate it, then you must brake its return. That eccentric phase is the heart of the device. At the Pôle Equilibre & Santé practice, Bloc A, 48 Rue de la Libération in Dudelange, the Desmotec D.PLUS and V.PLUS machines serve the tendon, the return of strength and return-to-sport work — never in a phase of uncontrolled acute pain.**\n\n## Accelerate, then decelerate\n\nUnlike a stack of weights, resistance follows the force you produce. The franker the acceleration, the more demanding the braking. It is that braking — muscle lengthening under tension — that interests a patellar tendon or a return after surgery, once the instructions are respected.\n\nTwo devices at the practice:\n\n- **Desmotec D.PLUS**: lower limb, closed-chain work, force sensors;\n- **Desmotec V.PLUS**: upper limb, open chain.\n\nThe software records force, speed, power, sometimes left–right balance. These figures dose the set; they do not rank patients.\n\n## Who benefits, and when\n\nSelected tendinopathies (notably patellar) as a complement to a loading programme; after ACL reconstruction or shoulder surgery once the initial phase has passed; return-to-sport and recurrence prevention in [sports physiotherapy](/dudelange/competence/kinesitherapie-du-sport); progressive return when someone is still too weak for a heavy fixed load.\n\nWe do not set up the flywheel on a sprain that is still very swollen, a hot knee that has not been explored, or a surgical limited-weight-bearing instruction that has not been lifted.\n\n## Typical session\n\nWarm-up, inertia setting (often low at the start), push and brake cues. Short sets, supervised recovery. If speed collapses or one side drops off, we adapt or we stop. Useful machine time: often **8 to 15 minutes** of effective work, within a wider session.\n\nSports kit, stable trainers. Prescription and post-operative instructions. Not on an empty stomach. Avoid a maximal workout the day before on the same muscle. Mention pain, swelling that day, dizziness or fever.\n\nAfterwards: DOMS for 24–48 h possible, normal if the dose is reasonable. Driving generally possible. Match or competition the same evening: no. Desk work: according to fatigue. Return to sport: strength, control and functional tests — the flywheel is only one tool among others, alongside [isokinetic testing](/dudelange/equipement/isocinetisme) when measurement is needed.\n\nResults: better braking capacity and confidence in weight-bearing, often within a few weeks of regularity. A tendinopathy is judged on pain with effort and tolerated load, not on the software score alone.\n\n## Contraindications\n\nUncontrolled acute pain, major swelling, fever, recent fracture, surgical instructions not respected, unexplored dizziness preventing balance. The assessment rules them out.\n\n## Frequently asked questions\n\n<details>\n<summary>How should I prepare and what should I wear?</summary>\n\nSports kit, trainers, prescription and weight-bearing instructions. Mention swelling, sharp pain or fever. Not on an empty stomach.\n\n</details>\n\n<details>\n<summary>How long on the flywheel?</summary>\n\nOften eight to fifteen minutes of effective work, dosed, within a session of about thirty minutes.\n\n</details>\n\n<details>\n<summary>Is it harder than a weight machine?</summary>\n\nEccentric braking is demanding, but inertia is set. We start low. This is not a competition.\n\n</details>\n\n<details>\n<summary>What if it hurts during the set?</summary>\n\nWe stop. Sharp tendon or joint pain is not to be “pushed through”. We reduce inertia or postpone.\n\n</details>\n\n<details>\n<summary>Can I drive and resume sport?</summary>\n\nDriving generally possible. Sport: according to strength, control and advice — not on the evening of a heavy set.\n\n</details>\n\n<details>\n<summary>What are the contraindications, and is it reimbursed?</summary>\n\nAcute phase, surgical instructions not lifted, fever, unexplored dizziness: no flywheel. On medical prescription, the [CNS](https://cns.public.lu/) covers the session according to the usual rules. The practice handles the paperwork.
