At Pôle Equilibre & Santé, in Bascharage, it is used mainly after surgery or a recent trauma, for comfort and oedema — not to accelerate healing.**\n\n## Four modes, manufacturer indications\n\nAccording to Game Ready, the Med4 Elite is indicated for post-surgical or acute oedema and pain. The four modalities:\n\n- **ice-free cold**, down to about **3 °C**;\n- **heat**, up to about **45 °C**;\n- **rapid contrast** (hot/cold alternation);\n- **intermittent pneumatic compression**.\n\nThe wrap fits the joint better than an ice pack. Cold reduces pain and local blood flow; compression helps drainage. The choice depends on the stage: cold is not a reflex three weeks after an operation if the knee is already cold, pale and stiff. Heat or contrast rather concern later stiffness.\n\n## Evidence: analgesia, not accelerated healing\n\nMarinova et al. (*KSSTA*, 2023): after total knee replacement, a Game Ready protocol improved **extension** more than ice with static compression, up to three months. Pain, flexion, circumference and opioids did not differ clearly. Useful for extension, in support of [orthopaedic rehabilitation](/bascharage/competence/reeducation-orthopedique) — not a wand against pain.\n\nBleakley, McDonough and MacAuley (*AJSM*, 2004): ice relieves pain in acute lesions; the evidence on oedema and return to sport remains limited.\n\nDubois and Esculier (*BJSM*, 2019), **PEACE & LOVE** framework: ice is an **optional analgesia**. It has not been shown to accelerate healing. We therefore do not claim that cold accelerates the healing of an injury.\n\n## How it goes\n\nAn adapted wrap (knee, ankle, shoulder, back…). Application often of about **20 minutes**. Temperature and pressure are set to stay comfortable. White toes or fingers, burning pain, excessive numbness: we raise the temperature or we remove the wrap. Cold burns exist.\n\nOften at the start of the session after an operation or a recent sprain, sometimes at the end after work that has woken the area.\n\n## Preparation\n\nClothing that allows the wrap to be placed; dry skin, without thick cream.\n\n## After the session\n\nDry, inspect the skin (transient redness is common; a persistent white patch or blisters: report it). Cold sometimes masks pain: this is not a green light for intense sport the same evening. Once inflammation drops, [thermotherapy](/bascharage/equipement/thermotherapie) and especially the exercises take over.\n\n## Contraindications\n\n**Raynaud’s syndrome**, a severe circulatory disorder, **loss of sensation** (neuropathy: you would not feel the burn), an **open wound**, a skin infection, cold allergy, cryoglobulinaemia. Grafted or very fragile skin: to be discussed. A coagulation disorder if compression is strong.\n\n## Frequently asked questions\n\n<details>\n<summary>Is it unpleasant? Can one burn oneself with the cold?</summary>\n\nLess than ice placed on bare skin. Intense cold is normal; burning pain, excessive numbness or white skin are not. The rhythm follows the stage (often more frequent the first days after surgery) then spaces. Never ice on bare skin, never all night. The interest of the Med4 Elite is the wrap, the compression and the controlled thermal dosing.\n\n</details>\n\n<details>\n<summary>When can I resume sport after a cold session?</summary>\n\nCold decreases pain: it does not validate a return. We resume according to swelling, weight-bearing, the surgeon’s instructions and tests.
